About the Job

Position Summary:  The Clinical Pharmacist Practitioner works collaboratively with other health care providers to provide patient-centered care for Lahey Clinic patients, with the goal of providing the highest quality care and ensuring the most optimal therapeutic outcome in a cost-effective manner.  This includes involvement in the collaborative care of patients with chronic diseases such as hypertension, hyperlipidemia, diabetes, asthma and COPD, as well as involvement in other multidisciplinary patient care activities; provision of drug information and education to both health care providers and patients; and involvement in Quality Measures initiatives. Additionally, the Clinical Pharmacist Practitioner assists with the continued development and promotion of clinical pharmacy services by participating in pharmacy staff, resident and student education, as well as supports the safe and effective use of medications through Pharmacy and Therapeutic Committee involvement and contribution to the pharmacy newsletter, as appropriate.   

Education: Must be a graduate of an accredited college of pharmacy with current Massachusetts registration in Pharmacy

Licensure, Certification, Registration: 

  • A pharmacy license in the State of Massachusetts
  • A Doctor of Pharmacy (Pharm.D.) degree, completion of an accredited residency program (preferably in the area of practice) or board certification in the area of practice, or equivalent education, training and experience functioning as a clinical pharmacist

Skills, Knowledge & Abilities:

  • Able to perform targeted physical exam for the purpose of pharmacotherapy monitoring
  • Proficient presentation and writing skills
  • Knowledge of Microsoft Office
  • Experience in precepting students and residents
  • Demonstrates the ability to be thoroughly trained to meet organizational standards.

 Experience:    Requires previous ambulatory care or direct patient care experience, as well a working knowledge of all basic aspects of health system pharmacy.
