About the Job

The Division of Cardiovascular Medicine at Lahey Hospital & Medical Center is seeking an Adult Advanced Practitioner for our expanding Heart Failure Program.  This is an exciting opportunity to work in collaboration with the Medical Director and staff of the multidisciplinary Heart Failure Program to provide advanced clinical services to both inpatients and outpatients with heart failure, cardiomyopathy and related diagnoses.  


The primary goals of the Advanced Heart Failure Program Inpatient Consultative Service are optimizing heart failure medical therapy, improving quality of life and reducing heart failure admissions through intensive education, follow up and coordination of care with Emergency Department, inpatient clinicians, specialists, post acute care providers and home care agencies.  The Inpatient Consultative Service team will be responsible for all aspects of heart failure care including comprehensive inpatient diagnostic evaluation, advanced therapies, management of outpatients with heart failure for the 30 day post discharge period utilizing  disease management including tele-management activities, patient/family education, palliative care and referrals as appropriate.    Education of the nursing staff regarding contemporary evidence based knowledge of heart failure treatment broadly and specific to individual patient is expected.     


Qualified APs will have a strong background in heart failure, cardiology and acute care medical management. 


In addition, this position will round on hospitalized HF patients and see patients triaged to HFP  consultative service within 7 days of discharge, as well as devise a customized management plan focusing on autonomy and self care promotion. This AP will also serve as a direct liaison to post-acute care providers.  The position will work closely with a multi-disciplinary team that includes Heart Failure Specialists, cardiologists, nurse practitioners, Palliative Care Specialists, pharmacotherapists, dietitians,  Heart Failure research nurse and a psychologist.

Licensure, Certification, Registration:


-Candidates must be graduates of a Master's Program in Nursing and must maintain currency through Clinical Education Units.

-Certification and current license to practice in the Commonwealth of Massachusetts

-ACLS certification

-DEA number and Mass Department of Public Health Division of Food and Drug

-Restricted Substances registration number as appropriate
