About the Job

Position Summary:

The Ambulatory Care Clinical Pharmacy Specialist works collaboratively with other health care providers to provide patient-centered care, with the goal of providing evidence-based, cost-effective pharmacotherapy for Beth Israel Lahey Health patients. The scope and nature of patient care services provided will depend on credentialing and privileging. The Ambulatory Care Clinical Pharmacy Specialist will have active involvement in interdisciplinary patient care activities, pharmacotherapy consultation, medication therapy monitoring, provision of drug information, education to both health care providers and patients, and guideline development and implementation. Additionally, the Ambulatory Care Clinical Pharmacy Specialist will assist with the continued development and promotion of clinical pharmacy services by participating in pharmacy staff, resident and student education, as well as support the safe and effective use of medication through Pharmacy and Therapeutic Committee involvement.

 

Essential Duties & Responsibilities including but not limited to:

Patient Care Activities:

  1. Collaborate with other members of the health care team to efficiently coordinate patient care
  2. Complete comprehensive medication histories and medication reconciliation
  3. Review and assess medication regimens and individual orders with focus on safety, efficacy, adherence and cost
  4. Adjust medications as permitted through collaborative practice agreement, where applicable
  5. Screen for drug interactions, therapeutic duplications, omissions, etc.
  6. Monitor patients for therapeutic and/or adverse response to medication
  7. Provide medication counseling to patients and caregivers
  8. Provide immunizations to patients and employees

Quality- and Cost-Focused Activities:

  1. Participate in the development and support of formulary management and medication utilization initiatives that promote the cost-effective use of pharmacotherapy for patients through involvement in the Lahey Pharmacy and Therapeutics Committee
  2. Participate in the development and implementation of evidence based treatment guidelines, order sets, and protocols
  3. Participate in the development and implementation of clinical and operational policies and procedures
  4. Educate health care professionals about drug therapy prescribing, management and monitoring
  5. Develop and participate in pharmacy-related quality improvement initiatives for patients
  6. Attend all required departmental meetings as well as other meetings as assigned
  7. Promote Lahey Health through participation in professional organizations or meetings and conferences
  8. Actively supports the quality improvement process through conducting medication use evaluations within the ambulatory care setting

Educational / Administrative Activities:

  1. Participate in the education and development of pharmacists, pharmacy residents and pharmacy students
  2. Participate in training of new employees and orientation of new health care providers where applicable
  3. Provide drug information and education to health care providers and patients/caregivers
  4. Document interventions and outcomes related to medication therapy management
  5. Maintain applicable certifications, competencies and continuing education
  6. Sustain outstanding skills as a self-directed clinical pharmacy specialist and consistently demonstrate the value of pharmacy services through maintaining updated knowledge of contemporary pharmacy practice, clinical skills and health care policy

Minimum Qualifications:

Education:

  • Must be a graduate of an accredited college of pharmacy

 

Licensure, Certification & Registration:

  • Must possess Massachusetts pharmacist license in good standing at time of hire 
  • Current Certification through the Board of Pharmacy Specialties (BPS) in applicable specialty or must obtain certification within the next 2 consecutive annual testing cycles after eligibility for such certification. Certification must be maintained throughout employment. If certification lapses during employment, recertification is required within the next annual testing cycle.
  • Current Basic Life Support (BLS) certification, or must obtain certification within the first year of employment. BLS certification must be maintained throughout employment.
  • Certification to administer immunizations or must obtain certification within the first year of employment.
  • Collaborative Practice agreements in core disease states when eligible within the first 3 years of employment.
  • Credentialed and Privileged by the Medical Staff Office when eligible within the first 3 years of employment.

 

Experience:

  • Requires a minimum of 2 years’ experience functioning as a clinical pharmacist in an applicable practice or completion of a post-graduate pharmacy residency focused in ambulatory care or applicable specialty.

 

Skills, Knowledge & Abilities:

  • Requires analytical skills to evaluate the appropriateness of drug therapy including patient assessment, drug therapy monitoring, and pharmacotherapy principles in resolution of patient care problems related to medication therapy management and making drug therapy recommendations.
  • Requires strong literature evaluation skills and understanding of principles of evidence-based medicine
  • Requires strong interpersonal and written skills to collaborate with all members of the healthcare team effectively to provide patient centered care
  • Must be self-directed, proactive, assertive, and possess creative problem solving skills.
  • Requires efficiency in use of Microsoft Office products, including Word, Excel and PowerPoint
