About the Job
Essential Responsibilities: Screens and triages patients who request service, are referred, or meet screening criteria. Responsible for psychosocial assessments, diagnosis, treatment and provides indicated services including crisis intervention, short term counseling, and group services. Assists in the adjustment and processing to new diagnosis, life altering illness or treatment. Assists patients and families in accessing resources including referrals to community agencies and treatment providers. Provides clinical intervention for issues of grief, depression, anxiety, caregiver stress, insomnia, goal setting for healthy life styles. Collaborates with physicians, nurses, external case managers, other community agencies and providers, and other health care professionals to implement patient care across the continuum. Assumes some regular responsibility for conducting seminars, primary supervision of social work staff and/or students; formal and informal interdisciplinary teaching and consultation. The successful candidate practices clinical social work at an advanced level independent of regular clinical supervision.
Prior experience working w/cognitive or movement disorder population a plus.
