Our team of nurses, therapists (physical, occupational, and speech), medical social workers, and home health aides work together to devise and implement a compassionate care plan that is expressly tailored for each patient.

About the Job

The Palliative Care Medical Social Worker is an integral member of the interdisciplinary palliative care team (IDT), and addresses psychological and social needs to support patients living with serious illness, and their families. The social worker provides comprehensive psychosocial care including completing psychosocial assessments and treatment plans, making regular home visits, providing supportive and/or grief counseling, assisting with patient/family advocacy and care coordination, facilitating referrals to community providers, and engaging families in emergency and advance care planning conversations. The social worker will assist patients and their families by promoting: coordination of care; education; patient advocacy; and efficient resource management.

Full time - 40 hours per week - day position

Minimum Qualifications:


  • A Master’s degree in Social Work (MSW) from an accredited school of social work .
  • Minimum of one year of social work experience in a health care setting.

Licensure, Certification & Registration:

  • Holds a current LCSW, and is licensed to work in Massachusetts. 
  • Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) preferred.
  • Maintains an American  Heart Association (AHA) CPR certification


  • Minimum of 3 years of Medical Social Work experience. 
  • At least 1-year experience in Palliative or End of Life Care.

Skills, Knowledge & Abilities:

  • Knowledge of Palliative and Hospice care.
  • Ability to evaluate and formulate assessments of psychosocial needs of individuals with complex medical conditions.
  • Ability to understand the mission of the Program and to implement its objectives for the delivery of patient care.
  • Interpersonal skills to interact effectively with medical staff, patients and families.
  • Facilitate follow up care meetings as needed with change of condition and/or as indicated by the care plan.
  • Understanding of end of life, long term care planning, Advanced Directives, and MOLST.
  • Demonstrates cultural competency and compassio
