About the Job
The Palliative Care Nurse provides consultative care services to palliative care patients with serious or life limiting illnesses with a focus on providing goals of care conversations and assisting patients in meeting those goals. Because these patients have a serious or life limiting illness, there will be a strong emphasis on communication and coordinating with the palliative team, other medical providers involved in the patient’s care, with hospitals, family members and caregivers.
