Our team of nurses, therapists (physical, occupational, and speech), medical social workers, and home health aides work together to devise and implement a compassionate care plan that is expressly tailored for each patient.

About the Job

The Nurse Practitioner supports and enhances clinical delivery with advanced practice assessment of high risk, wound and palliative care patients.  Provide clinical education to staff and managers related to this group of patients. 

Full time, 40 hour, day position covering the Milton area


  • Perform assessments on patients who are determined to be high risk in an effort to prevent rehospitalizations.
  • Perform palliative care evaluations for patient and their families
  • Perform Face to Face assessments for hospice patients to determine need for recertification.
  • Attend hospice IDT on a bi weekly basis
  • Coordinate and present in-service programs when indicated.
  • Function as a liaison between clinical staff, physicians and community resources as directed
  • In conjunction with the Executive Director of Hospice and Palliative Care, assist in the development of programs which will enhance the role of the Nurse Practitioner

Minimum Qualifications:


  • Graduate of National League for Nursing accredited school of nursing. 

Licensure, Certification & Registration:

  • Nurse Practitioner with current licensure to practice in the state of Massachusetts.
  • Maintains a current American  Heart Association (AHA) CPR certification


  • 5+ years of clinical experience with a minimum of 2 years in community health required 

Skills, Knowledge & Abilities:

  • Demonstrates excellent observation and problem solving/critical thinking skills.
  • Well-developed verbal and written communication skills
  • Essential computer skills to work with EMR.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Self-directed and able to work with minimal supervision
  • Shows strong ability to organize and prioritize workload independently; nursing skills per competency checklist
