About the Job
Position Summary:
This Outpatient Specialty Pharmacist is responsible for optimizing medication management for patients being treated in the oncology infusion clinic, with a focus on patients utilizing specialty pharmacy medications. The pharmacist will interact with patients to gather medication histories, assess adherence, and provide medication education. The pharmacist works closely with the multidisciplinary care team and patient to facilitate access to and/or modify medication therapy to overcome barriers. Knowledge of chemotherapy regimens and confidence in interacting with physicians to discuss treatment plans is necessary. Understanding of specialty pharmacy and prior authorization processes is desired. The pharmacist will also serve as a drug information resource for nurses and providers to meet the operational and clinical needs of the infusion clinic. Additionally, this position will collaborate with the Pharmacy Leadership Team to assist with the continued development and promotion of clinical pharmacy services, as well as support the safe and effective use of medication through multidisciplinary committee involvement.
Scheduled Hours
40 hours per week, day shfit.
