About the Job
Candidates must possess the ability to meet our services excellence standards along with excellent clinical knowledge and a desire to be a part of a fast paced, multi-disciplinary team. Utilizes the case management process to identify the specific needs of the patient in order to ensure that hospital services are covered for the patient, in addition to developing a safe and effective discharge plan.
· Establishes a therapeutic relationship with the patient, the patient's family and significant others by involving them in each step of the care coordination process and accommodates factors that may influence the plan of discharge.
· Collaborates with the multi-disciplinary healthcare team regarding the patient’s plan of care to promote the best possible outcomes and patient/family experience upon discharge.
· Ability to learn utilization management principles and hospital leveling processes for compliance and insurance purposes
QUALIFICATIONS:
Education:
Required: BSN is required
EXPERIENCE
Required - At least three (3) years of home health and/or post-acute care experience.
Preferred - Previous Case Management experience
Other Skills/Knowledge
Required
Excellent interpersonal skills and the ability to work collaboratively with a provider staff as well as the rest of the case management team.
For safety and quality reasons, must be able to read, write and communicate effectively in English with patients, visitors, vendors, and fellow members of the hospital team.
