About the Job

Transitional Support Services (TSS) is a program dedicated to helping men and women by providing short-term residential support services to individuals in early recovery from drug and/or alcohol addiction. Clients are provided with the tools they need to begin developing skills of daily living including goal setting, problem solving, and self-advocacy. Services include nursing support, individual case management, group treatment sessions, aftercare planning, and more. The Case Manager role is a great opportunity for an individual who wants to advocate for and serve individuals who struggle with addiction within a collaborative, established, patient-centered organization.

The Senior Case Manager's Responsibilities Are:

  • Provides support to case managers and interns including monitoring case load, removing aftercare barriers and assigning new clients as appropriate.
  • In collaboration with the clinical supervisor, reviews all clinical documentation on a quarterly basis to assure full compliance with Federal, State, and Program regulations and stipulations.
  • Works with the clinical supervisor to develop a group schedule and ensure group material is derived from evidence-based practice.
  • In collaboration with the leadership team, conducts ongoing review of the clinical program design and implements recommendations for improvement.
  • Works with clients whose complexity requires increased administrative/clinical intervention and assessment. Communicates clinical complexities with the treatment team and develops individualized treatment plan for each client.
  • Participates, when requested, in the hiring process and scheduling of staff. Provides performance guidance and disciplinary concerns to the clinical supervisor and program director.
  • Attends and participates in Leadership Team meetings, Program Team meetings, Clinical Supervision meetings, and other meetings as required.
  • Collaborates with outside referral sources to ensure clients have confirmed aftercare placement
  • Assisting case management with facilitating MAT transfers and courtesy dosing, assuring a seamless transition between levels of care.


  • Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required.
  • A minimum of one year related experience and/or training in the area of addiction treatment.
  • Valid Driver’s License in good standing preferred.
