About the Job
Position Summary: Medical Social Worker
Responsibilities include: Working closely with treatment team, will perform psychosocial assessments, supportive counseling and care coordination to ensure safe and successful discharge transitions for adult and geriatric Senior Adult Unit (SAU) patients and families. Will serve as SAU liaison with colleagues in the BILH system as well as providers of senior oriented services in the community to enhance knowledge of SAU services and assist in identifying patients who could benefit from an SAU admission.
40 hours per week, day shift.

At least one year related experience. Experience with protective services, long term care, crisis intervention, chronic illness, hospice care, substance use disorders and psychiatric illness preferred. Acute care experience and familiarity with local senior services.
A candidate will have successfully completed a Master's degree in Social Work
Current Massachusetts License in Social Work
