About the Job
The Homecare Referrals Coordinator is responsible for the smooth transition of the patient into the home health environment. Receives and coordinates referrals to Beth Israel Lahey Health at Home from affiliated and outside physicians, hospitals, facilities and other community referral sources.
Part time position - every Sat and Sun 9:30 a.m. - 6:00 p.m. - 16 hours work for 24 hours pay.
