COPD/Pneumonia APP Job Description Posting for HR


LHMC Division of Pulmonary and Critical Care Medicine seeks a highly motivated and committed Advanced Practice Provider (APP) for a position dedicated to our population health initiative in pulmonary medicine focusing on COPD and pneumonia patients. The APP will be responsible for identifying  patients admitted to the hospital with COPD exacerbations and/or pneumonia, providing in-patient consultation on a daily basis, organizing multifaceted patient management, and operationalizing comprehensive outpatient follow up.  Additional responsibilities will include management of the patient care lists for this population, providing patient care recommendations, directing colleagues in proper documentation, and providing post hospital follow up care. Clinical support will be provided by a faculty Pulmonary physician in the in-patient and outpatient setting.


Applicants should be highly motivated, confident and patient centric team players who have a strong interest in Pulmonary medicine, with a particular focus on acute and post acute care management of patients hospitalized with COPD and pneumonia. Organized, dedicated advanced practitioners with time management and adaptability skills are highly desirable.
