About the Job

The Behavioral Health Community Partner (BHCP) Program is a new and innovative care management program designed for eligible MassHealth ACO members. The program aims to engage individuals with high behavioral health needs and provide them with a care plan that utilizes a person-centered approach to healthcare. The RN Care Manager is a critical part of the care team and is a highly administratively functioning nursing role. This position is a great opportunity for someone who wants to make a difference in the Behavioral Health field.

The RN Care Manager’s Responsibilities Are:

  • Performs medication reconciliation, ensuring that the client  understands and adheres to medication administration, oftentimes coordinating with the enrollee’s PCP
  • Reviews comprehensive assessments including medical history, medical needs, medication and functional status
  • Collaborates with the BHCP care team to ensure that assigned enrollee’s behavioral and physical care needs are met and coordinated across the continuum of care
  • Assists in identifying supports that are available or could be made available to support enrollee in medication administration and compliance
  • Provides health education, coaching and symptom management to enable enrollee to manage their chronic condition(s)
  • Participates in training, clinical rounds, quality meetings, and treatment team meetings
  • Works effectively with community based resources, referral networks, and public/private systems of care
  • Perform other duties as needed to support the BHCP program requirements & specifications

Qualifications:

  • Valid and current Massachusetts licensure as a Registered Nurse
  • BLS Certification required
  • Experience in the field of mental health and/or substance abuse preferred
  • Experience with Excel, Word, and electronic health records preferred

Schedule:

This is a full-time, 40 hours/week, benefit-eligible positon. The anticipated schedule is Monday through Friday, day shift.
