Lahey Hospital & Medical Center is seeking a Lic. Clinical Social Worker to support the Kidney and Liver Transplant Department. We are seeking clinician to support the inpatient setting following transplantation and ambulatory clinic as needed. Successful candidate will conduct initial evaluation and subsequent regular follow-up assessments of transplant recipients functioning, compliance, adjustment to transplantion and psychosocial needs. Provide interventions as identified in assessment to address any psychosocial problems/barriers that interfere with post-transplantation plan of care. Provide counseling, psycho-education, groups, support, crisis intervention and other psychosocial services to patients and their families in order to help them cope with psychosocial stresses related to end stage Kidney or Liver disease and adjustment to Transplantation. Assist patients through advocacy procedures and referrals to community agencies as indicated. Experience working with end stage liver or kidney disease a plus.
Qualifications: Skilled in the areas of psychosocial assessment, treatment/intervention and coordination of care. Able to provide a variety of treatment modalities; including individual, couples, group and family therapy. Utilizes a theoretical base of knowledge to assess, diagnose, formulate interventions and evaluate clinical outcomes. Candidate should be able to prioritize and manage caseload within a fast paced multi-disciplinary setting.
Education: Graduate of an accredited Master’s School of Social Work.
Licensure, Certification, Registration: Currently licensed at the LCSW level by the Massachusetts Board of Licensure for Social Work. Shift: 8-4:30p
