About the Job

Position Summary:  Under the direct supervision of the Anti-Coagulation Program Manager and the general supervision of the Administrative Director and Anticoagulation Program Medical Director, the Anticoagulation pharmacist will be instrumental in facilitating the safe and effective use of anticoagulation therapy at Lahey Clinic.  Critical elements of this position include providing patient, staff and provider education; performing therapeutic monitoring for efficacy and safety; evaluating for potential drug interaction; making recommendations regarding appropriate dosage adjustment; and effectively communicating to patients, families and other members of the healthcare team.  In collaboration with other anticoagulation team members, the Anticoagulation Program Clinical Pharmacist will participate in the development, maintenance and direction of the Thrombosis Service; ensure appropriate, complete and accurate documentation in the Patient's Medical Record; perform the daily operations including data collection and analysis, and identifying performance improvement opportunities.  


Essential Duties & Responsibilities including but not limited to:

  1. Clinically reviews anticoagulation medication orders to identify patients receiving therapeutic doses of anticoagulation medications.  Assessment should include, appropriate drug selection, medication allergy, dose, route of administration, indication for anticoagulation, relevant laboratory results, therapeutic duplication, drug interactions and age of patient.
  1.  Intervenes with appropriate health care professional when necessary, and documents clinical interventions consistent with departmental policy.
  1. Appropriately enters dosing recommendations and other pertinent communications regarding the anticoagulation therapy into the Patient’s Medical Record. 
  1. Provides clinical guidance related to anticoagulation therapy to Nurses, physicians, Interns, medical students, and other members of the health care team.  Participates in the Pharmacy Residency program as a clinical preceptor.  
  1. Provides anticoagulation drug information to medical staff and patients upon request or when need is apparent.  Documents request into the intervention database in accordance with departmental policy
  1. Responsible for day-to-day evaluation of patients on anticoagulation therapy to identify patients requiring close monitoring, evaluation of drug-drug interactions, monitoring of therapeutic lab results and communicating recommended adjustments to therapy to the appropriate provider managing the patient’s care.
  1. Responsible for understanding and utilizing all technology to accomplish the above.
  1. Serves as a liaison between the Anticoagulation Department and any health care provider
  1. Pharmacists are responsible for their own professional development including, meeting all requirements for continuing education, keeping abreast of current professional literature, both from a clinical and medication safety perspective.  Pharmacists are expected to participate in select pharmacy department activities such as the clinical staff meeting and clinical conferences.
  1. Actively participates in continuous performance improvement activities of the department, including providing suggestions on how to improve day-to-day operations
  1. Performs assigned projects in a efficient and timely manner
  1. Demonstrates competency in all practice areas in which Anticoagulation Pharmacist duties are performed.  Has completed all required departmental competency requirements.
  1. Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals  into daily activities.

Minimum Qualifications:


Education:  Must be a graduate of an accredited college of pharmacy with a current MA registration in Pharmacy


Licensure, Certification, Registration:  Must possess a valid MA pharmacist license

Skills, Knowledge & Abilities:  Demonstrates the ability to be thoroughly trained to meet the requirements of this job description.


Experience:     Previous hospital pharmacy experience is preferred.  Excellent communication, interpersonal skills and teamwork is required.  Residency experience or board certification or equivalent  experience preferred.
