About the Job
Position Summary: Under the direct supervision of the Anti-Coagulation Program Manager and the general supervision of the Administrative Director and Anticoagulation Program Medical Director, the Anticoagulation pharmacist will be instrumental in facilitating the safe and effective use of anticoagulation therapy at Lahey Clinic. Critical elements of this position include providing patient, staff and provider education; performing therapeutic monitoring for efficacy and safety; evaluating for potential drug interaction; making recommendations regarding appropriate dosage adjustment; and effectively communicating to patients, families and other members of the healthcare team. In collaboration with other anticoagulation team members, the Anticoagulation Program Clinical Pharmacist will participate in the development, maintenance and direction of the Thrombosis Service; ensure appropriate, complete and accurate documentation in the Patient's Medical Record; perform the daily operations including data collection and analysis, and identifying performance improvement opportunities.

Minimum Qualifications:
Education: Must be a graduate of an accredited college of pharmacy with a current MA registration in Pharmacy
Licensure, Certification, Registration: Must possess a valid MA pharmacist license
Skills, Knowledge & Abilities: Demonstrates the ability to be thoroughly trained to meet the requirements of this job description.
Experience: Previous hospital pharmacy experience is preferred. Excellent communication, interpersonal skills and teamwork is required. Residency experience or board certification or equivalent experience preferred.
