About the Job

Working with the Director of Resident Care oversees Resident clinical management, personal care and education program participation in accordance with EOEA regulations as well as The Herrick House policies and procedures. 

Essential Job Functions:  The employee must be able to perform these essential functions with or without reasonable accommodation by The Herrick House. Works as part of a multi-disciplinary team to provide care and services to Residents and families and support to staff. Must be proficient in the Point Click Care system.


M-F; 2 weekend days/month

  • Organizes Resident pre-move activities which include working with the Resident and/or family, their physician and any provider to obtain current history and physical, MD orders, advanced directives, insurance information, health care proxy documents, demographic sheet and medical clearance form.
  • Develops the initial service plan for new Residents.
  • Reviews Service Plans with Resident and family.
  • Conducts 30 Day Meetings with Resident/family and multidisciplinary team.
  • Does Service Plan reassessments every six months or with change in Resident condition.
  • Oversees RCA documentation for all three shifts.
  • Conducts quality assurance checks on timeliness of e-call responses.
  • Facilitates daily update meetings on Spectrum Memory Care Residence.
  • Integral member of the Quality Assurance process, assisting in audits, SAMM/RCA reviews every 6 months.
  • Participates in fire and elopement drills routinely.
  • Facilitates in-service programs.
  • Conducts orientations and onboarding of new staff.
  • Participates in bi-weekly Tracking Meeting.
  • Maintains confidentiality of all Resident communications.


Education/Experience: Minimum of two years’ experience working with individuals with dementia diagnoses.


Current LPN Massachusetts License
