About the Job
This position will implement, monitor and evaluate telehealth (TH) and disease management programs. This role will coordinate services for patients receiving tele-monitoring, which includes assessing patient appropriateness and monitoring of patient data. Will work with clinicians, managers and physicians to ensure quality care and desired outcomes. Reports client data to MDs and clinical managers, and prepares reports for analytical purposes. This position will assist in the development and implementation of policies and procedures with regard to the telehealth programs. Other duties may be assigned as the program evolves.
