Our team of nurses, therapists (physical, occupational, and speech), medical social workers, and home health aides work together to devise and implement a compassionate care plan that is expressly tailored for each patient.

About the Job

Manages total operations of hospice house residence including but not limited to:

  1. The patient admission process: 
    • Works with referral sources to determine appropriateness and priority of patients to be admitted.
    • Ensures accommodations area ready for the patient and loved ones so they feel welcomed and comforted in the organization's preparedness and professionalism.
    • Ensures the patient record and all other systematic processes are performed accurately and efficiently
  2. Manage Hospice House staff and volunteers:
    • Hire, orient, train and mentor new staff.  Manage performance and career growth with regular and formal performance feedback.  In consultation with Human Resources, disciplines and terminates staff as necessary.
    • Ensures licensed staff are trained and reviewed by qualified clinicians.
    • Develops the orientation and training program for all residential staff; documents the successful completion of a training and orientation program; ensures all residential staff has the necessary skills to provide the prescribed service.  Collaborates with resources to provide more specialized and/or clinical training.
    • Builds a collaborative, safe and enjoyable work environment to improve recruitment and retention and encourage volunteerism from the community.
    • Supervises, trains and manages performance of volunteers.
    • Supervises of the Hospice Volunteer Coordinator for both residential and home visiting hospice programs
    • Determine adequate and appropriate staffing:
      • Creates and maintains working schedule of licensed caregivers, case manager, personal care attendants and volunteers based on resident’s acuity and census to ensure proper staffing ratios.
      • Manages staffing mixes and schedules within the personnel budget.
  3. Facilities maintenance and aesthetics;
    • Ensures the house's heating, air conditioning, electricity, phone services, landscaping and snow removal and other infrastructure issues are properly maintained to ensure safety and comfort
    • Creates aesthetically pleasing environment for patients, loved ones, referral resources and potential clients which includes cleaning, supply chain and inventory management of other amenities and facility operational issues.
    • Assesses and supports family presence; determines appropriateness of utilizing family rooms and makes decision accordingly.
  4. Business and Community Development
    • Collects and analyzes census data and other patient statistics  to build short and longer term strategic and operational plans for the House
    • Cultivates relationships with potential referral resources to ensure sources and individuals choose our services.
    • Seeks out and attends events and other opportunities intended to build the business and good will in the community.
    • Run community education presentations as determined by the Vice President of Operations for Homecare & Hospice.
    • Conducts or delegates patient/family tours of the Hospice House residence and provides supervision of new patient orientation of the residence
    • Develops and implements a philanthropy program for the House.
      • Teaches staff how to address grateful patient opportunities.
      • Solicits donations and planned giving opportunities.
      • Builds relationships in the community to create philanthropic opportunities.  Solicits donations of various types to meet the short and long term needs of the House.
  5. Takes responsibility for the operational and business success of the Hospice house. 

Performing and/or delegating duties to ensure safe, quality care that is fiscally responsible and compliant with any and all laws, regulations and clinical guidelines

Qualifications:

A Bachelor's degree in a relevant field.  An additional eight years of relevant experience may be considered in lieu of the degree. Clinical experience preferred. 5+ years in direct management of employees, Experience with facilities and/or equipment management. Experience creating working 24 hour schedule of multi-faceted employees. 

Home Care Assisted Living experience or Hospice experience required

Maintains a current American Heart Association (AHA) BLS certification.

Excellent leadership qualities and staff development skills. Strong organization, planning, problem solving and decision-making skills. Computer literate with a working knowledge of Microsoft Office. Ability to analyze issues and develop solutions. Well-developed written and oral communication skills. Ability to meet deadlines and manage multiple tasks. Ability to work independently and pro-actively. Ability to maintain a professional appearance and image. Strong business acumen. Results driven. Ability and willingness for local travel within the community. Strong communication and collaborative skills with a demonstrated ability to speak, write, and communicate in English. Must demonstrate the ability to interact in a positive and courteous manner with patients, families, other employees and outside vendors. Strong interpersonal skills that include tact and diplomacy.   A detail orientation and the ability to work both independently and as part of a team are essential.
