About the Job
Position Summary:
The Hematology Oncology Clinical Pharmacist Specialist works collaboratively with other health care providers to provide patient-centered care, with the goal of providing evidence-based, cost-effective pharmacotherapy for Lahey Health patients. The scope and nature of patient care services provided will depend on the practice setting, but typically would include active involvement in interdisciplinary patient care activities, pharmacotherapy consultation, medication therapy monitoring, provision of drug information, education to both health care providers and patients, and guideline development and implementation. Additionally, the Clinical Pharmacy Specialist assists with the continued development and promotion of clinical pharmacy services by participating in pharmacy staff, resident and student education, as well as support the safe and effective use of medication through Pharmacy and Therapeutic Committee involvement. |

Minimum Qualifications:
Education: | Preferred PGY2 or 3 years equiv. |
Licensure, Certification & Registration: | Must possess MA pharmacist license in good standing at time of hire . |
Experience: | Hematology Oncology Ecperience |
Skills, Knowledge & Abilities: | Requires analytical skills to evaluate the appropriateness of drug therapy including the patient assessment, drug therapy monitoring, and pharmacotherapy principles in resolution of patient care problems related to medication therapy management and making drug therapy recommendations. Requires strong interpersonal skills to collaborate with all members of the healthcare team effectively to provide patient centered care.
