About the Job

Position Summary:

The Hematology Oncology  Clinical  Pharmacist Specialist works collaboratively with other health care providers to provide patient-centered care, with the goal of providing evidence-based, cost-effective pharmacotherapy for Lahey Health patients. The scope and nature of patient care services provided will depend on the practice setting, but typically would include active involvement in interdisciplinary patient care activities, pharmacotherapy consultation, medication therapy monitoring, provision of drug information, education to both health care providers and patients, and guideline development and implementation. Additionally, the Clinical Pharmacy Specialist assists with the continued development and promotion of clinical pharmacy services by participating in pharmacy staff, resident and student education, as well as support the safe and effective use of medication through Pharmacy and Therapeutic Committee involvement.


Essential Duties & Responsibilities including but not limited to:

Quality Focused Activities:


  1. Promote Pharmacy involvement in the collaborative care of Lahey Health Patients.
  2. Promote Lahey Health through participation in professional organizations or meetings and conferences.
  3. Utilize practice management skills to maintain cohesive, cost-efficient and productive care of patients and relationships with other clinicians.
  4. Participate in formulary management and review of medication use processes through involvement in the Pharmacy and Therapeutics Committee.
  5. Participate in the development and implementation of evidence based treatment guidelines, order sets and protocols through involvement in the Standing Orders Committee.

Clinical Functions:


  1. Provide evidence-based, patient-centered medication therapy management with interdisciplinary teams, which may include collaborative drug therapy management agreements with other health care providers per policy, where applicable.
  2. Develop and participate in programs focused on optimizing patients’ therapeutic outcomes.
  3. Document interventions and outcomes related to medication therapy management activities in pharmacy and electronic medical record systems.
  4. Provide drug information and education to health care providers and patients/caregivers

Educational/Other Functions: 


  1. Participate in the education and development of pharmacists, pharmacy residents and pharmacy students. Act as a preceptor for pharmacy residents and as a project advisor in area of expertise or as appropriate..
  2. Provide support for the pharmacy newsletter through written articles and promotion of publication.
  3. Obtain, within the first year of employment, immunization training certification , and participate in immunization programs targeting patients and employees.
  4. Obtain, within the first year of employment, basic life support (BLS) certification, and maintain such certification throughout employment..
  5. If not Board of Pharmaceutical Specialties (BPS) certified upon employment, obtain Board Certification in Pharmacotherapy or specialty area of practice (if available through BPS) within the next 2 consecutive annual testing cycles after eligibility to obtain such certification (3 consecutive annual testing cycles if hired after May 1), and maintain such certification throughout employment. If certification lapses during employment, recertification is required within the next annual testing cycle.

  7. Participates in departmental and/or interdepartmental quality improvement activities.
  8. Participates in and successfully completes Mandatory Education.
  9. Performs other duties in support of the organization as required or directed.



Minimum Qualifications:


Preferred PGY2 or 3 years equiv.


Licensure, Certification & Registration:

Must possess MA pharmacist license in good standing at time of hire  .

Skills, Knowledge & Abilities:

Requires analytical skills to evaluate the appropriateness of drug therapy including the patient assessment, drug therapy monitoring, and pharmacotherapy principles in resolution of patient care problems related to medication therapy management and making drug therapy recommendations.  Requires strong interpersonal skills to collaborate with all members of the healthcare team effectively to provide patient centered care.
