About the Job
Education: Master’s in Social Work
Required: At least five (5) years as a clinical/medical social worker
Experience with a variety of populations
Ability to work as a part of a multi-disciplinary team in a busy emergency room.
Demonstrated ability to effectively apply professional principles and contribute to the successful management of patients
Assess patients to determine psychosocial needs, ensuring a safe transition home or to the next level of care
Patient coverage will include inpatient medical units as well as ICU and Emergency Department.
Provide therapeutic intervention to help support and empower patients in achieving positive health outcomes.
Function as a part of the case management team as well as the emergency services team.
Participate in complex care planning and assume responsibility for meeting the unique needs of patients.
For safety and quality reasons, must be able to read, write and communicate effectively in English with patients, visitors, vendors, and fellow members of the hospital team
LICENSES, REGISTRATIONS, CERTIFICATIONS:
Required: Commonwealth of Massachusetts Licensed Social Worker
