About the Job
Position Summary: Under the supervision of the manager and working within their scope of practice as part of a multidisciplinary team, the Physical Therapist II assesses, plans, organizes, and participates in rehabilitative programs that improve or restore physical function, alleviate pain and prevent dysfunction in patients with impairments, functional limitations, disabilities or changes in physical function and health status from injury, disease or other causes. The Physical Therapist II acts as a clinical and administrative resource for the department and helps to facilitate safe and effective flow of department functions while maintaining the requirements of a Physical Therapist II as described in the “Rehabilitation Services Career Ladder Document”.
Essential Duties & Responsibilities including but not limited to:
1)Patient Care Examination:
§Examines patient using appropriate testing methods and instrumentation, appropriately records and interprets all findings, and identifies patient needs.
§Formulates a detailed assessment based upon findings of the examination.
2)Treatment Planning and Intervention:
§Safely and effectively implements the treatment intervention. Applies sound clinical judgment and skill.
§Engages a high degree of patient motivation and cooperation in treatment, as evidenced by patient compliance with treatment intervention.
§Provides patient/family education during therapy and in preparation for discharge.
§Conducts ongoing assessments of the patient’s status and revises treatment intervention, goals & outcomes as appropriate.
4)Patient Care Documentation:
§Thoroughly and accurately maintains all medical record documentation in accordance with the department policy and procedures.
§Documents the initial evaluation, daily treatment notes, reassessments and discharge summaries in the medical record according to the standards defined in the department policy and procedure.
5)Productivity / statistics
? Develops and maintains physical therapy skills and knowledge per the Lahey Rehabilitation Services Career Ladder as described in the Rehabilitation Services’ Career Ladder document.
? Meets qualifications for a Physical Therapist II every two years as described in the document “Rehabilitation Services Career Ladder”.
? Documents Achievements: must maintain an ongoing record of activities throughout the year.
Education: BS Physical Therapy from an accredited program.
Licensure, Certification, Registration: Active MA Physical Therapy license.
