About the Job

The Nurse Practitioner supports and enhances clinical delivery with advanced practice assessment of high risk, wound and palliative care patients.  Provide clinical education to staff and     managers related to this group of patients.

ESSENTIAL FUNCTIONS:

  Perform assessments on patients who are determined to be high risk in an effort to prevent rehospitalizations.

  Perform assessments on wound care patients to determine appropriate treatments which will provide favorable outcomes.

  Perform palliative care evaluations for patient and their families

  Perform Face to Face assessments for hospice patients to determine need for recertification.

  Attend hospice IDT on a bi weekly basis

  Coordinate and present in-service programs when indicated.

  Function as a liaison between clinical staff, physicians and community resources as directed

  In conjunction with the Executive Director of Hospice and Palliative Care, assist in the development of programs which will enhance the role of the Nurse Practitioner

Qualified candidates will be a graduate of an accredited school of nursing and a current Massachusetts NP license with:

  5+ years of clinical experience with 2 years in  community health required

  BSN preferred

  AHA BLS certification required
