About the Job
The Department of Transplantation and Hepatobiliary Diseases at Lahey Hospital and Medical Center, is seeking a talented Transplant Nurse Coordinator. The Transplant Nurse Coordinator is responsible for providing nursing care, education and support to all patients receiving care in the Department of Transplantation and Hepatobiliary Diseases. Their primary role is focused on organ donors and transplant candidates, patients on the pre-transplant waiting list, post-transplant patients both in the hospital and in the community. A Transplant Nurse Coordinator collaborates in the delivery of health care services with other team members including physicians, advanced practitioners, pharmacy, social service, behavioral medicine, nutrition, radiology, physical therapy, laboratory and case management. The Transplant Nurse Coordinator provides telephone triage for pre and post-transplant patients and live donors. Patient, family, and nursing colleague education is an integral part of the transplant nurse coordinator’s role.
The Transplant Nurse Coordinator is responsible for the quality of nursing care he/she provides to individuals and groups. Responsibilities include health maintenance, teaching, counseling, collaborative planning and restoration of optimal functioning and comfort for the dignified death of those they serve.
In addition, the Transplant Nurse Coordinator assumes the responsibility for the plan of care and has the authority to make decisions. The Transplant Nurse Coordinator is held accountable for decisions specific to assessment, planning, implementation and evaluation of the plan. The Transplant Nurse Coordinator facilitates the coordination of care across the continuum and demonstrates safe practice within the domains of:
• Clinical judgment and decision making
• Professional relationships
• Clinical leadership
• Clinical scholarship
Education: BSN Preferred
Licensure, Certification, Registration: Current Massachusetts state nursing license. Current
certificate from American Heart Association (AHA) for Basic Life Support (BLS).
Experience: Two years of transplant related nursing experience preferred
