About the Job
The RN Hospice Case Manager is responsible for the management of hospice and palliative care of patients and families assigned to a caseload in accordance with hospice regulation and payer guidelines.
Registered Nurse, a Graduate of an accredited school of nursing with current licensure to practice professional nursing in the state of Massachusetts. BSN strongly preferred. One to two years of Medical/Surgical Nursing experience; oncology/hospice experience preferred. Maintains a current American Heart Association (AHA) CPR certification.
Demonstrates excellent observation and problem solving/critical thinking skills. Well-developed verbal and written comunication skills; essential computer skills to work with EMR. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. Self-directed and able to work with minimal supervision. Shows strong ability to organize and prioritize workload independently, nursing skills per competency checklist.
Part time, 32 hours per week position, day shift
