The CSP Program Assistant is responsible for providing outreach case management services and coordination of treatment to a clinically diverse population. In addition to direct client service provision, the CSP Program Assistant will assist the Program Director with multiple administrative tasks to support the program.
Essential Functions/Key Responsibilities:
1. Assumes an individual case load, completes a comprehensive needs assessment of the client within two weeks of the initial contact with the client.
2. Develops an individualized and solution focused service plan with clearly defined interventions and measureable outcomes. These plans are developed with the client, CSP worker, other treatment providers and family/guardian, significant other with proper consent. These plans are reviewed and revised at least every 90 days.
3. Provides community based case management services to assist clients in accessing further treatment, self-help groups, housing and services related to physical health and other activities of daily living.
4. Provides temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.).
5. The CSP worker reports to the outpatient clinician, as needed basis concerning changes in behavior, unusual incidents or admission to detox or other non-preauthorized levels of care.
6. Coordinates with other providers and collaterals and navigates the benefit/entitlement system, including managing insurance authorizations.
7. Provides for or helps the client and his/her family access education, educational materials on mental health and substance use disorders and treatment.
8. Ensures that all documentation is complete in a timely manner.
9. Ensures that all billing documentation is complete in a timely manner.
10. Participates in discharge planning from CSP. The CSP worker will update the crisis prevention plan and develop a written aftercare plan. Discharge from the CSP program will occur in consultation with the client and referral source when discharge criteria are met.
11. Filing and conducting quarterly record compliance audits/records management.
12. Assists the Program Director with taking requests for services, checking insurance eligibility, obtaining insurance authorizations, and opening/tracking new cases.
13. Assists the Program Director with reconciling and submitting staff time sheets and billing.
14. Other related duties as assigned.
Listens and gets clarification, responds well to questions and demonstrates group presentation skills.
Writes clearly and informatively. Working knowledge and use of Microsoft Outlook (email/calendar), Microsoft Word and Excel computer software. Ability to respond to common inquiries or complaints from consumers, regulatory agencies, or other service providers. Ability to define problems, collect data, establish facts and draw valid conclusions.
Bachelor's Degree in psychology, social work or related human services field preferred, but not required. In lieu of a bachelors degree, must have one to three years training or related experience in the areas of mental health and/or addiction/recovery. Experience delivering community based services is a plus.
Certificates, Licenses, Registrations, LADC/CADC preferred.
Demonstrates good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from mental illness and/or substance use, as applicable. Demonstrates the ability to evaluate what is needed by each individual and adjust approach accordingly. Have in-depth knowledge of local resources. Experience with accessing resources and navigating mental health and/or substance abuse treatment systems a priority. Demonstrates excellent communication, documentation, time management and organizational skills.
40 hours, full-time.
Must be able and willing to transport persons served in personal vehicle. As such, must have a valid driver's license, reliable vehicle, and ability to satisfy LHBS approved driver policy. Travel and an ability to meet persons served in a variety of outreach settings is required.
