Recovery Coach, Full or Part Time available

Lahey MassHealth ACO (LMH-ACO) is responsible for improving coordination of patient care, reducing unnecessary hospitalizations and readmissions, emergency department visits and revisits, and reducing total cost of care over time. LMH-ACO will provide services in collaboration with contracted Managed Care Organizations as well as Community Partners for Behavioral Health and Long Term Services and Supports, as appropriate.

The Recovery Coach, LMH-ACO, provides non-clinical support to individuals with substance use disorder (SUD), including but not limited to connecting the individual with recovery support services, serving as a recovery guide and role model in the management of recovery, assisting the individual to identify and overcome barriers to recovery, providing support with problem solving and advocacy, and educating the individual and family members on the recovery process.

Essential Duties & Responsibilities (including but not limited to):
- Understands the SUD service system and supports navigation of treatment and recovery resources
- Acts as an advocate for the needs of the individual
- Provides education on the signs and symptoms of an overdose, overdose prevention and the use of naloxone to the individual and offer the individual an overdose prevention kit
- Provides telephonic, text, and in-person support, coaching, and assistance to help the individual access treatment or recovery support services
- For patients not yet in treatment, conducts a warm handoff of the individual to appropriate treatment or recovery services
- Demonstrates an understanding and belief in the recovery process and Recovery Oriented Systems of Care Model
- Has knowledge of basic crisis intervention, Motivational interviewing, and case management techniques preferred.
- Demonstrates good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from a SUD
- Demonstrates the ability to evaluate what is needed by each individual and adjust approach accordingly.
- Has in-depth knowledge of local resources.
- Provides temporary assistance with transportation as needed to essential behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.)
- Coordinates with other members of the care management team
- Demonstrates collaborative and professional approach to supporting patients in accessing appropriate healthcare services
- Completes other related duties as assigned


Education: High school diploma required; bachelor degree preferred

Licensure, Certification & Registration: NA

- Peer with lived addiction experience
- At least 2 years of current and sustained recovery without reoccurrence
- Proficient in computer skills
- Information technology and electronic medical record experience preferred
- Experience with MassHealth population preferred

Skills, Knowledge & Abilities:
- Driver?s license and car required
- At least 2 years of current and sustained recovery without reoccurrence
- Completion of the BSAS Recovery Coach Academy or the Connecticut Community Addiction Recovery (CCAR) within 3 months of hire
