Family Support and Training is delivered by strength-based, culturally and linguistically appropriate qualified paraprofessionals under the supervision of a licensed clinician. It is a service provided to the parent /caregiver of a youth (under the age of 21), in any setting where the youth resides, such as the home (including foster homes and therapeutic foster homes), and other community settings.
Family Support and Training is provided within a structured, one-to-one, strength-based peer relationship between a Family Partner and a parent/caregiver. The purpose of this service is for resolving or ameliorating the youth?s emotional and behavioral needs by improving the capacity of the parent /caregiver to parent the youth. This is to improve the youth?s functioning as identified in the Family System Intervention (FSI) service IAP or clinical hub service IAP, for youth enrolled in Flexible Support Services, and to support the youth in the community or to assist the youth in returning to the community.
Services may include education, assistance in navigating the child serving systems (DCF, DMH, education, mental health, juvenile justice, etc.); fostering empowerment, including linkages to peer/parent support and self-help groups; assistance in identifying formal and community resources (e.g., after-school programs, food assistance, summer camps, etc.) support, guiding, coaching, and training for the parent/caregiver.
1. Work with family towards meeting goal(s) established in an existing behavioral health treatment plan, for youth enrolled in the Individual and Family Flexible Support Services program under: FSI, IYS, and/or YSG.
2. Support the parent/caregiver?s capacity to ameliorate or resolve the youth?s emotional or behavioral needs and strengthen their capacity to parent.
3. Work one-on-one and maintain regular frequent contact with the parent(s)/caregiver(s) in order to provide education and support throughout involvement within the IFFSS program or as deemed eligible by the Department of Mental Health. 1:1 meetings may occur in the office, the family's home, or another community location.
4. Educate parents/ caregivers about how to effectively navigate the child-serving systems for themselves and about the existence of informal/community resources available to them; and facilitates the parent?s/caregiver?s access to these resources.
5. Coordinate and communicate with members of the FSI team to support the treatment goals and the identified safety plan.
6. Complete timely and accurate documentation to reflect services provided.
7. Staff person can only bill for services rendered and those that are billable according to the specifications.
8. Person carries a caseload as assigned.
9. Communicate with the Department of Mental Health to provide timely updates about services provided.
10. Meet productivity standard for hours worked.
11. Build and adjust schedule as necessary to meet the needs of persons served. Afternoon/evening hours are required. Staff must be willing to schedule and hold appointments on weekends based upon the needs of the families served.
12. Uphold values and mission of the Children's Behavioral Health Initiative.
13. Other related duties as assigned.
Minimum staff qualifications for a Family Partner includes:
Experience as a caregiver of a youth with special needs, and preferably a youth with mental health needs; bachelor?s degree in a human services field from an accredited university and one (1) year of experience working with the target population; or associate's degree in a human service field from an accredited school and one (1) year of experience working with children/adolescents/transition age youth; or high school diploma or GED and a minimum of two (2) years of experience working with children/adolescents/transition age youth; and experience in navigating any of the child and family serving systems and teaching family members who are involved with the child and family serving systems.
Part time, 20 hours per week
Must be able and willing to transport persons served. As such, all qualified candidates must have a valid driver's license, good driving record, and reliable vehicle. Travel and an ability to meet persons served in a variety of outreach settings are required.
