The CSP Worker is responsible for providing outreach case management services and coordination of care to a clinically diverse population.
1. Assumes an individual case load, completes a comprehensive needs assessment of the client within two weeks of the initial contact with the client. These assessments are reviewed by supervisor and revised at least every 90 days.
2. The CSP worker obtains biopsychosocial assessment from outpatient clinician and/or referring source, within two weeks of the initial contact with the client, and incorporates it into comprehensive needs assessment. The CSP worker consults with outpatient clinician, on an as needed basis, concerning changes in behavior, unusual incidents or admission to detox or other non-preauthorized levels of care.
3. Develops an individualized and solution focused service plan with clearly defined interventions and measureable outcomes, within 2 weeks of the initial contact with the client. These plans are developed with the client, CSP worker, other treatment providers and family/guardian, significant other with proper consent. These plans are reviewed by supervisor and revised at least every 90 days.
4. Provides community based case management services to assist clients in mitigating barriers to accessing further treatment, self-help groups, housing and services related to physical health and other activities of daily living.
5. Provides temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.).
6. Coordinates with other providers and collaterals to navigate the benefit/entitlement system, including managing insurance authorizations.
7. Directly provides, or helps the client and his/her family access, education and/or educational materials on mental health and substance use disorders and treatment.
8. Participates in discharge planning from CSP. The CSP worker will update the crisis prevention plan and develop a written aftercare plan. Discharge from the CSP program will occur in consultation with the client and referral source when discharge criteria are met.
9. Ensures that all required documentation is complete in a timely manner.
10. Ensures that all billing documentation is complete in a timely manner.
11. Other related duties as assigned.
Bachelor's Degree in psychology, social work or related human services field preferred, but not required. In lieu of a bachelor?s degree, must have one to three years training or related experience in the areas of delivering community based services and addiction/recovery. Certificates, Licenses, Registrations, LADC/CADC preferred.
Demonstrates good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from mental illness and/or substance use. Demonstrates the ability to evaluate what is needed by each individual and adjust approach accordingly. Have in-depth knowledge of local resources. Experience with accessing resources and navigating mental health and/or substance abuse treatment systems a priority. Demonstrates excellent communication, documentation, time management and organizational skills.
40 hours per week - full-time
Must be able and willing to transport persons served. As such, must have a valid driver's license, good driving record, and reliable vehicle. Travel and an ability to meet persons served in a variety of outreach settings is required.
