The Advanced Practitioner (NP or PA) for the optimization clinic/Orthopedic co-management operates under the departments of Anesthesia and Hospital Medicine. The NP/PA while working in the optimization clinic will be under the direct supervision of the Director of Perioperative Services and work in direct collaboration with the Chair of Anesthesia and supervising anesthesia physicians. He/she will perform duties to assist with the optimization of patients undergoing a surgical procedure. While working in the inpatient units on medical co-management for post-surgical patients, the NP/PA will work under the clinical supervision of the Division Chair of Hospital Medicine and supervising hospital medicine physicians. He/she will evaluate post-operative patients to ensure a smooth transition of medical issues post-operative and consult hospital medicine when additional medical expertise is needed to ensure the best plan for the patient. As a licensed independent practitioner, the nurse practitioner participates as a team leader and member in the provision of health and medical care, interaction with professional colleagues to provide comprehensive care.
Functions independently to perform age and/or procedure specific history and physical for complex acute, critical, and chronically ill perioperative patients.
Orders and interprets diagnostic and therapeutic tests relative to a patient?s specific needs.
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
Implements and monitors effective interventions to support the patient to regain or maintain physiologic stability for perioperative preparation and postoperative clinical management.
Facilitates the patient?s transition within the health care setting.
Rounds on post-operative patients to ensure a smooth care transition in the post-surgical period
Works with hospital medicine physicians for medical conditions that require consultation and additional medical expertise
Collaborates with multidisciplinary team members by making appropriate referrals.
Facilitates staff, patient and family decision making by providing educational tools.
Possess exceptional organizational skills, multitasking ability, and effective communication skills to ensure patients and care providers are well informed.
The Advanced Practitioner is a licensed independent practitioner who provides primary and/or specialty nursing and medical care in ambulatory, acute and long term care settings. The NP is a registered nurse with specialized advanced education and clinical competency to provide health and medical care to diverse populations. Masters, post masters or doctoral preparation is required for entry level practice. (AANP 2006)
