Hospice RN Case Manager, Lahey Health at Home, Full TimeJob ID 147815 Date posted 05/05/2018
Responsible for the management of hospice and palliative care of patients and families assigned to caseload
Current license to practice as a registered professional nurse in the Commonwealth of Massachusetts. One to two years of Medical/Surgical Nursing experience; oncology/hospice experience preferred.
Strong communication and collaborative skills with a demonstrated ability to speak, write, and communicate in English. Must demonstrate the ability to interact in a positive and courteous manner with patients, families, other employees and outside vendors. A detail orientation and the ability to work both independently and as part of a team are essential.
