About the Job

Position Summary: The Licensed Practical Nurse performs professional nursing care, functions as a member of the hospice team and provides input into the assessment, planning, and evaluation of nursing services. Under the direction and supervision of the RN Case Manager - Hospice, the LPN is accountable for the quality of nursing care he or she provides and assumes responsibility for the provision of nursing care in accordance with the established plan of care for assigned patients.

Responsibilities

Essential Duties & Responsibilities including but not limited to:

Patient Care

  1. Performs nursing visits to patients as delegated/assigned by the RN Case Manager – Hospice /Hospice Clinical Manager.
  2. Provides hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes as directed by the RN.
  3. Observes, records, and reports significant changes in the patient’s healthcare status to the MD and RN Case Manager – Hospice.
  4. Evaluates outcomes of basic nursing interventions, reports and encourages necessary changes to the plan of care.
  5. Participates in the health teaching required by the patient and family.
  6. Respects and supports patient and family’s cultural beliefs and values, individual needs, health goals and treatment preferences.

Communication

Effectively communicates with patients, caregivers, and members of the interdisciplinary healthcare team incorporating interpersonal and therapeutic communication skills.

  1. Regularly collaborates with the RN Case Manager – Hospice regarding the patient’s status and needs to ensure quality and continuity of nursing care.
  2. Communicates with the physician to report significant changes in the patient’s condition; obtains/receives physician’s orders as required.
  3. Attends regular IDT meetings.
  4. Practices confidentiality principles set by the agency and federal HIPAA guidelines.

Documentation

  1. Clinical visit notes reflect an accurate and detailed description of the patient and situation and communicate skilled care.
  2. Completes documentation in accordance with agency timeliness policies.

Professional Development

  1. Maintains and updates clinical knowledge and skills based on current nursing practice.
  2. Adheres to departmental requirements for required training.
  3. Identifies specific learning needs and goals and collaborates with nursing leadership in developing a plan to meet them.

Additional Duties

  1. Participates in the weekend and holiday rotation as defined by Beth Israel Lahey Health at Home.
  2. Ensures that arrangements for equipment and other necessary items and services are available.
  3. Participates in in-service programs.

  • Participate in and successfully completes Mandatory Education.
  • Perform all other duties as needed or directed to meet the needs of the department.

Qualifications

Minimum Qualifications:

Education:

  • Graduate from a Licensed Practical Nursing program accredited by the National League of Nursing (NLNAC).

Licensure, Certification & Registration:

  • Current licensure to practice as an LPN in the state of Massachusetts.
  • Maintains CPR certification.

Experience:

  • Two years of experience as a Licensed Practical Nurse in an acute care, rehab or home care setting.
  • One year of hospice experience preferred.

Skills, Knowledge & Abilities:

  • Demonstrates excellent observation and problem solving/critical thinking skills.
  • Well-developed verbal and written communication skills; essential computer skills to work with EMR.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Self-directed and able to work with minimal supervision
  • Shows strong ability to organize and prioritize workload independently; nursing skills per competency checklist.
