About the Job

Experienced APP with current certified diabetes care and education specialist (CDCES) certification. with CDCES working full time 40 hrs/week. This position will be primarily based in the outpatient clinic with potential for future inpatient rotation.

The APP will work in collaboration with 12 endocrinologists, including supervising physician according to approved practice guidelines and scope of practice agreed upon by APP and supervising MD

Patients can alternate visits between APP and MD or be seen primarily by the APP based on clinical scenario.

Accepting referrals and consults from within Lahey Hospital and Medical Center, BILH affiliates, and the surrounding community.

The APP will collaborate with multiple specialties as needed for optimal patient care.

The APP will participate in precepting students as well as provider and staff education nurse when requested.

The practice population consists of individuals with diverse cultural, ethnic and language backgrounds often requiring translation services using in person translators, telephone or video services. lThe APP will diagnose, evaluate and manage care of people with gestational diabetes, type 1 and type 2 diabetes, thyroid disease, osteoporosis, and hypogonadism age 18 and older.

The APP will average 12 patients per day, allowing 30 minute visits for follow-up appointments and 60 minute visits for new patient appointments. The APP may also assist patient with continuous glucose monitor training, insulin pump and advanced technology education. In addition, the APP may provide diabetes self-management education as indicated. Video visits are possible for MA patients with diabetes.

The APP will initiate and manage diabetes medications and therapies including treatment for hyperlipidemia, HTN, and neuropathy pain. Complicated patient presentations are reviewed with patient’s endocrinologist or referred to appropriate specialties: ex: nephrology, neurology, behavioral health, gastroenterology, podiatry, ophthalmology, and cardiology.

Performing history and targeted physical exam, ordering and interpreting appropriate diagnostic tests,reviewing results with patient, records and documents assessment of data, intervention,s, reulsts and patient outcomes in ERH, filling prescriptions, completing forms and writing letters as needed, working with administrative support team to meet patient and clinic need.
